Community Health Work
We have leveraged community based education foundations to provide relationship centered caregiving focused on improving outcomes from Social Determinants of Health for individuals in South Dakota. Limited access to resources with those barriers often labeled as individual failings rather than systemically driven. Our team focuses on meeting and supporting individuals where they are, with specific focus within the identities of Native American women, single parents, and food service workers in accessing the robust network of social care in South Dakota. Our pillars of education, support, and connection guide us as we endeavor to transform how these services are perceived by populations who have had experiences that discourage their full participation. We accomplish this via community health workers who are already embedded within these communities.

Our agency members reside in the community they serve, equipping them with the unique ability to bring information where it is needed most.
